Charge Conference Forms
All Charge Conference Forms must be completed and submitted electronically using the 2026 Charge Conference Access Page, prior to your Charge Conference. If you have any questions, please contact your District Administrator.
Instructions
Open the Charge Conference Access Page and provide your name, email address and the name of your church. Then select the form(s) you need to complete. You can select one form at a time, so if you need to complete multiple forms, use the Access Page to generate each form separately.
When you choose a form from the Access Page and click "Submit," your requested form will open in a new browser tab. The information you entered will automatically be filled in for you, as well as the district and the church email address, if available.
Please complete the forms making sure that all required fields are filled in. If you need more time, you can choose to save your progress and continue later. If you choose to save your progress, you will get an email with a link to complete the form at a later time.
When you are finished with a form and submit it, a copy of the completed form will be emailed to you and to the church email address. The district and conference will receive a copy of all submitted charge conference forms.
Equalization
Equalization is the process by which we determine and account for clergy and lay members of Annual Conference. Click to view a listing of the number of lay members to be elected by each charge conference within the district for the 2027 meeting of the Horizon Texas Conference in Denton, June 10-13.
The procedure for determining clergy/lay equalization is listed on pages 62-63 of the 2026 Preliminary Workbook.
