Charge Conference Forms

All Charge Conference Forms must be completed and submitted electronically using the 2025 Charge Conference Form Checklist, prior to your Charge Conference. If you have any questions, please contact your District Administrator.

Instructions

Open the Charge Conference Form Checklist and provide your name, email address and the name of your church. Then select the form(s) you need to complete. You can select one form at a time, so if you need to complete multiple forms, use the Checklist to access each form separately.

When you choose a form from the Checklist and click "Submit," your requested form will open in a new browser tab. The information you entered on the checklist will automatically be filled in for you, as well as the district and the church email address, if available.

Please complete the form making sure that all required fields are filled in. If you need more time, you can choose to save your progress and continue later. If you choose to save your progress, you will get an email with a link to complete the form at a later time.

When you are finished with the form and are ready to submit it, the form will be saved and can be viewed by the district and the conference offices. A copy of the completed form will be emailed to you and to the church email address.
